Complaints

If you have any queries or concerns about your treatment, please talk to your own GP first.

If you wish to make a complaint regarding any aspect of the practice or the care that you have received, this should be put in writing to the Practice Manager using our Patient Complaint Form.

Further information about the practice’s Complaints Procedure is detailed in the following Complaints Procedure.

If you wish to make a complaint on behalf of another person, you will need to also complete our Patient Complaint Third Party Consent Form.

Copes of both the complaints form and leaflet are available in hard copy from reception.
