If you wish to make a complaint regarding any aspect of the practice or the care that you have received, this should be put in writing to the Practice Manager using our Patient Complaint Form.

If you wish to make a complaint on behalf of another person, you will need to also complete our Patient Complaint Third Party Consent Form.
